Texas gunman Devin Kelley escaped from a mental health facility years before his assault on a small-town church-and he had made threats that he would kill.

The Air Force confirmed Monday that it failed to transmit records of Kelley's conviction to an Federal Bureau of Investigation database used for background checks for gun purchases, and officials ordered an internal review of the case and Air Force policies. If that had been done, Kelley could have been stopped from purchasing the assault rifle he used in the shooting.

The air force has opened an inquiry into the matter, and the U.S. Defense Department has requested a review by its inspector general to ensure other criminal cases have been reported correctly, Pentagon officials said.

Kelley was found guilty by court-martial in 2012 of assaulting his first wife and a stepson while serving at Holloman Air Force Base, where he was assigned to a logistics readiness unit, the Pentagon reported on Monday.

The sheriff said he was trying to determine how deputies came to believe Kelley had moved and why they did not continue to pursue the case, either in Colorado or after Kelley returned to the area later. They spoke of the gunman's anger against his estranged wife's mother who usually attends the First Baptist Church in Sutherland Springs, but was not present at the time when he went on the rampage on Sunday.

Officials on Tuesday praised the Good Samaritan neighbor, Stephen Willeford, who shot Kelley in the leg and the torso outside the church and pursued him out of town, calling him a "hero". "There was a domestic situation going on within the family and the in-laws", Freeman Martin of the Texas Department of Public Safety told reporters. 10 people are in a critical condition.
